WPO PROGRAM ELEMENTS

BEST PRACTICE 2

Utilize evidence-based, ongoing assessment processes for individuals entering the program with well-defined expectations for participation.

Example of how this Best Practice connects to the Guiding Principles:

Address trauma: programs are person-centered and individualized.

Build and support community: WPOs facilitate connections to other services.

Examples of how organizations can implement this Best Practice:

  • WPOs standardize internal practices to ensure all staff who work with an individual can share information and build on each other's assets.

  • WPO staff intentionally build relationships with individuals participating in the program.

  • WPO staff intentionally build relationships with and are educated about other services, resources, and programs in the community to facilitate good referrals and warm handoffs.

  • Long-term, WPOs work toward coordination across the system to share information, facilitate hand-offs, and ensure individuals are matched with the best program to meet their needs.

  • Utilize evidence-based assessments to gather necessary and appropriate information about individuals.
